Decomposition of the growth of health care expenditures under National Health Insurance and Medical Aid in Korea

Abstract
This study aims to analyze expenditure profile by age group and to decompose the growth of health care expenditures under National Health Insurance (NHI) and Medical Aid and to identify the impact of population aging on expenditures in particular. We used the data on number of enrollees, visit days and expenditures for each 5-year age group of the NHI and Medical Aid from 2003 to 2012. Examination of expenditure profile by age group reveals that health care resources are allocated more for older population over time in NHI while relatively homogeneous profile across age groups in Medical Aid. However, results from decomposition analyses present that changing in population structure accounts for 20.8% of the increases in NHI compared with 7.8% in Medical Aid. Findings from this study suggest that price factor is most significant determinant of growth of health care expenditures than population aging. Furthermore, volume factor is important for inpatient care and price factor for outpatient care in both NHI and Medical aid.
